Closing the Gaps in Patient Care Plans: Executive Roles

Preventable patient harm in five of the largest hospitals in one county of southeast Texas exceeded $235M (2015-16) with significant unfavorable impact on operating margins. In the quest to improve healthcare, increasing attention is being paid to gaining control over quality by making care safe, effective, timely and centered on patient needs. The complexity of healthcare delivery can benefit from the care team having access to real-time insights into patient safety threats/risks to prevent adverse events before they occur. Incorporating predictive risk management technology into the care plan can be an effective means for preventing harm. Hospital leadership must create an environment that invites and enables care teams to succeed through the application of High Reliability Organization (HRO) principles.

Integrating Acute to Post Acute Care Settings: Where Do We Go From Here?
Management of populations of patients through integrated post acute care models can not be done by a simple referral. As our populations change, so must the delivery of healthcare services along the care continuum. Acute Care Hospitals are faced with ever more regulatory requirements and expanding financial penalties all in an effort to elevate the Standard of Care across the healthcare system on a national scale. These regulatory requirements and the corresponding financial penalties are not new phenomena. Most hospitals aggressively develop performance improvement plans that center on the resolution of these deficits for minimization of potential financial exposure. We are now in a new wave of financial penalties to Acute Care Hospitals with the Medicare payment reductions for returns to acute within 30 days of discharge.

FACHE Exam Prep Session will be held February 8.  There is no cost to attend this prepatory class for the Board of Governor’s Exam.

The Chapter will hold its annual reception the evening of February 85-6 pm at the MHA facility.  This is an excellent opportunity for you to network with your peers from around the state.

The Early Careerist Award will be presented by the Mississippi Regent, Christopher L. Morgan, FACHE.
